What edition is your part? I have the parts from IMSLP (scans of a recent Kalmus edition?) and it looks like all three questions you ask are corrected in this edition:
1. beat 3 is an e minor chord.
2. both hands play a fifth, D-A.
3. LH is in bass clef.
